Student-Athlete Information

Physicals:

Student athletes must complete a physical prior to participation in their sport. It is recommended that athletes attend the pre-participation physicals provided by Allan Hancock College. The on-campus physicals are free of charge.

If an athlete is unable to attend the on-campus physicals they have the option of scheduling an off-campus physical.

The following forms need to be filled out by the student athlete, their guardian and/or physician prior to attending the on-campus physicals. They include a medical questionnaire, screening form, emergency contact and insurance information and more. Thank you!
