2020-21 Preparticipation Physical Examination packet for Student Athlete Guide

                Welcome Back to Spring 2021!!

If you were on campus in the fall (2020), it is mandatory to complete the following before we can allow you on campus again:

3. Attend the mandatory COVID-19 team Return to Socialization Zoom Education when scheduled.

4. Mandatory COVID-19 Return to Campus Quiz (Must pass Quiz – see below)

5. Complete the Pre-Participation Forms packet (see link below).  Sign and submit.

6. Physical Examination (see link below).  Complete, Sign, Print, Physical by Doctor and Submit.

   • It’s preferred to submit after a physical via Abode Scan to Cheo at emunoz@hancockcollege.edu
7. Provide us with a copy of your Primary Medical Insurance.  Please email copy to emunoz@hancockcollege.edu after submitting your Pre-Participation Exam packet.

8. If you are a minor, your parent/guardian will need to fill and sign.

9. Mandatory Concussion Testing.  This will be scheduled with your team when on campus.  (Review).
