Athletic Training Forms

1. SportsWare

All student-athletes are required to create a SportsWare account for athletic training records and reports. 

2. Physical Forms

3. Minor Consent Form

If you are a minor, defined as any individual under the age of 18 by the State of California, a parent/legal guardian is required to complete the Minor Consent Form. Please click the button below to access the Minor Consent Form.

4. Insurance Verification Form

All student-athletes are required to provide proof of insurance prior to competing in an athletic event at AHC. Please click the button below to complete the insurance verification form.
